1. Visit the website of  Chief Electoral Officer, Madhya Pradesh.
2. Click on the “Search Name in Voter List”.
3. Use your EPIC number or Voter ID to search for your name in the list.
4. If you don't have your EPIC with you, you can still search for your name using other options like your phone number or personal details.
5. You can use other details such as your date of birth, the district you're in, your assembly area, and details about a relative to find your name on the voter list.

How to Apply for Madhya Pradesh Voter ID Card Online?

Step 1: Access the Portal

• Go to the National Voters' Services Portal (NVSP) website.

Step 2: Initiate Sign-Up

• Click on the 'Sign-Up' option located at the top right corner of the homepage.

Step 3: Enter Your Details

• Provide your mobile number, email ID, and captcha code, then select 'Continue'.

Step 4: Set Up Your Account

• Input your 'First Name', 'Last Name', 'Password', and 'Confirm Password', then click the 'Request OTP' button.

Step 5: Verify Your Identity

• Enter the OTP received on your mobile number and email ID, and select 'Verify'.

Step 6: Log Into Your Account

• Click the 'Login' button, enter your mobile number, password, and captcha, then press the 'Request OTP' button.

Step 7: Complete Login

• Input the OTP received on your mobile number and select the 'Verify & Login' button.

Step 8: Start New Voter Registration

• Select the 'Fill Form 6' button found under the 'New registration for general electors' tab.

Step 9: Fill Out Form 6

• Complete all required fields on Form 6, including personal, relatives', and contact details, Aadhaar information, date of birth, and address. Attach necessary documents as instructed and click the 'Preview and Submit' button.

Step 10: Finalize Submission

• Review all details for accuracy and click 'Submit' to complete your registration.

How to Apply for Voter ID Card in Madhya Pradesh Offline? 

Here are the steps to apply for a Voter ID card through the Electoral Office:

• Visit the Nearest Electoral Office: Go to the Electoral Office closest to your location.
• Request Form 6: Ask the officer for Form 6, which is the application form for a new Voter ID card.
• Complete Form 6: Fill in all the necessary details as required on Form 6.
• Attach Required Documents: Gather all the supporting documents needed for the application and attach them to Form 6.
• Submit the Form: Submit the completed Form 6 along with the attached documents at the Electoral Office.
• Verification by Booth Level Officer: A Booth Level Officer may visit your home to verify the details provided in the application.

• One recent passport-size photograph.

• Residency Requirement: The individual must be a regular resident of the constituency for which they are registering to vote.

How to Check Voter ID Status in Madhya Pradesh? 

1. Visit the National Voter Services' Portal: Go to the official National Voter Services' Portal website.
2. Log In: Click the 'Login' button and enter your mobile number, password, and captcha code. Then, click the 'Request OTP' button.
3. Verify Your Identity: Enter the One-Time Password (OTP) you received and click the 'Verify and Login' button.
4. Track Your Application Status: Select the 'Track Application Status' tab.
5. Enter Your Details: Input the reference number from the acknowledgement slip you received when submitting your voter registration application. Select "State as Madhya Pradesh" and click the 'Submit' button.
6. Check Your Status: Your current voter registration status will be displayed on the screen.

How to Download the Madhya Pradesh Voter ID Card Online? 

Step 1: Visit the Portal

• Go to the official National Voter Services’ Portal to start the process.

Step 2: Initiate Login

• Click on the ‘Login’ button located typically at the top right corner of the page.

Step 3: Authenticate

• Enter your login credentials followed by the OTP (One-Time Password) sent to your registered mobile number to securely log in.

Step 4: Access e-EPIC Services

• Once logged in, click on the ‘E-EPIC Download’ tab which you can find on the homepage.

Step 5: Choose Identification Method

• Select whether you wish to proceed by entering your ‘EPIC No.’ or ‘Form Reference No.’

Step 6: Search Your Record

• Input either your EPIC number or the reference ID number. Make sure to select 'Madhya Pradesh' as the State and then click on the ‘Search’ button.

Step 7: Request OTP

• Your Voter ID details will be displayed. Click on the ‘Send OTP’ button. Please ensure that your mobile number is updated in the NVSP records. If necessary, update your mobile number by submitting ‘Form 8’ on the NVSP website.

Step 8: Verify OTP

• Enter the OTP you receive on your registered mobile number and click the ‘Verify’ button.

Step 9: Download e-EPIC

• After OTP verification, you will be able to click on the ‘Download e-EPIC’ button to download your digital Voter ID.

How to Update Address in Voter ID in Madhya Pradesh Online? 

• Log in to the official National Voters Services Portal.
• Navigate to the 'Shifting of Residence/Correction of Entries in Existing Electoral Roll' section on the homepage.
• Click on 'Form 8' to be redirected to a new page; enter required details.
• Choose the option 'Self' and click 'Submit'.
• Select 'Shifting of Residence', then choose either 'Within Assembly Constituency' or 'Outside Assembly Constituency' and click 'Ok'.
• Select your 'State, District, and Assembly/Parliamentary Constituency' and click 'Next'.
• Provide your Aadhaar number, email, and mobile number, then click 'Next'.
• Input your new address and the required supporting documents.
• Upload the supporting document and click 'Next'.
• Complete the declaration, enter the captcha code, and click 'Submit'.
• The Election Commission of India will then process your request, and, upon approval, update your Voter ID card accordingly.

How to Update Address in Voter ID in Madhya Pradesh offline?

1. Go to your nearest Electoral Office.
2. Request and collect Form 8.
3. Fill in all the required information on Form 8.
4. Submit Form 8, along with proof of residence document, to the nearest Electoral Office.
5. Wait for your application to be processed and verified.
6. Once your details are confirmed, you will receive a new Voter ID card.

How to Apply for Madhya Pradesh Duplicate Voter ID Card Online/ Offline? 

Step 1: Obtain Form EPIC-002

• Visit the Election Commission of India's website to download 'Form EPIC-002'.
• Alternatively, you can collect a physical copy of the form from your nearest Electoral Office.

Step 2: Fill Out the Form

• Complete 'Form EPIC-002' with all the required details, including your name, age, relative's name, address, and existing Voter ID (EPIC) details.

Step 3: Submit the Form

• Submit the form at your nearest Electoral Office.

After Submission
