It is an application to get registered on the electoral list. Any Indian citizen who is 18 years old or above can fill out form 6 to apply for Voter ID. If you are leaving the constituency where you are currently registered as a voter, you can also use this form to register yourself in a new constituency.
Follow the below steps to fill up Voter ID card Form 6 for online registration:
Step 1: Go to the official Voters’ Service portal.
Step 2: Click on ‘Sign-up’ and enter the required details to register on the portal.
Step 3: Log in to the Voters’ Service portal by entering your mobile number and password.
Step 4: Under ‘Forms’, click the ‘Fill Form 6’ button.
Step 5: Enter all the information on Form 6, i.e., your state, district, parliamentary constituency, personal details, relatives details, contact details, Aadhaar details, gender, date of birth, address and declaration.
Step 6: Upload all the necessary documents on the Form.
Step 7: Click the ‘Preview and Submit’ button. When all the details on the form are correct, click on ‘Submit’.
After this, the system will generate an acknowledgement number, which you can use to track your application.

You can file a Form 6 application at any point of time during the year. Moreover, if you wish to submit this form offline, you can do so at the offices of your constituency's Assistant Electoral Registration Officer or Electoral Registration Officer.
