  • Form 2: This is a common PF nomination form typically used for fund amount payments to your respective nominees or nominee.

Required Information to Fill PF Form 2

Under the Employees’ Pension and PF schemes, with Form 2, you can add a nominee to your EPF account. All you need to do is fill out the specific form by entering several details. This includes:

  • Name of the member
  • Date of birth
  • Father name
  • Permanent Address
  • Name of the nominee
  • Date of birth of the nominee
  • Complete address
  • Member’s relationship with the nominee
  • Portion of the PF amount that is to be paid to nominees
  • If the member does not have any dependent family or parents as described in para 2(g) of the Employees provident fund Scheme 1952, then he/she should remove the sections that do not apply.
  • If the nominee is a minor, a member must provide information about the guardian who will receive the PF amount. 
  • Signature of the subscriber
  • Information about family members who will receive the children or widow’s pension when the member dies. 
  • Signature of employer/authorised officer
  • Name and address of the factory or establishment.

Required Information to Fill PF Form 11

Form 11 requires certain details that are listed below:

  • Name of the member, date of birth, mobile number, father's/husband's name, and email ID.
  • If he/she is a member Employees Provident Fund Scheme, 1952 and Employees’ Pension scheme 1995, details regarding the previous employment are required. This includes date of exit, previous pension payment order and previous scheme certificate number.
  • International workers must mention details related to passport number, country of origin, passport validity, marital status, education qualification, and any disabilities.
  • KYC details including Aadhaar, PAN, Voter ID, Driving License, etc.
  • Signature of the employer and establishment’s seal
  • Declaration by the present employer.

Required Information to Fill PF Form 9

A member is required to provide the following information for Form 9:

  • Name of the member, date of birth, mobile number, father's/husband's name, and email ID.
  • In the form, if you mention that you were a member of the Employees Provident Fund Scheme, 1952 and Employees’ Pension scheme 1995, then you need to fill up details. This includes UAN or previous PF account ID, previous pension payment order, previous scheme certificate number, etc.
  • KYC details including Aadhaar, PAN, Voter ID, Driving License, etc.
  • International workers must mention details related to passport number, country of origin, passport validity, marital status, education qualification, and any disabilities.
  • Declaration by the present employer. 
  • Signature of employer and seal of the establishment.

  • Ensure your gateway profile picture, DOB, and current and previous address is up to date. Otherwise, you will not be able to edit your EPF nominee online.
  • Your mobile number and Aadhaar should be linked.
  • Your Aadhaar card’s name, DOB, and gender should match your EPF records.
