Here's how to fill Form 6 for online registration on the NVSP:
Step 1: Visit the official National Voters’ Service portal.
Step 2: Click on ‘Sign-up’/‘Log-in’ and enter the required details to register on the portal.
Step 3: Under ‘New Voter Registration’, click on the ‘Fill Form 6’ button.
Step 4: Enter all the information on Form 6, i.e., your state, district, parliamentary constituency, personal details, relatives details, contact details, Aadhaar details, gender, date of birth, address and declaration.
Step 5: Upload all the necessary documents.
Step 6: Click the ‘Preview and Submit’ button. When all the details on the form are correct, click on ‘Submit’.
An acknowledgement number is generated after submission for tracking.
Form 6 can also be filled and submitted through the official Voter Helpline App:
Step 1: Download and install the Voter Helpline App from the Google Play Store or Apple App Store.
Step 2: Register or log in using your mobile number and OTP.
Step 3: On the dashboard, select ‘New Voter Registration’. Then select Form 6.
Step 4: Enter required details such as personal information, address, and constituency.
Step 5: Upload supporting documents and photograph.
Step 6: Review the entered details and submit the application.
Step 7: Note the acknowledgement/reference number to track the application status.
The app also allows tracking of Form 6 status and downloading of e-EPIC after approval.
Step 1: Download Form 6 from the Voters’ Services Portal. Alternatively, the same can be obtained at the Electoral Registration Officer (ERO) office, Assistant Electoral Registration Officer (AERO) office, Booth Level Officer (BLO), or designated polling stations.

Step 4: Attach self-attested documents for DOB and address proof.
Step 5: Sign the declaration section.
