How to Apply for Ayushman Card Online?

The Ayushman Card enables eligible beneficiaries under Ayushman Bharat PMJAY to access cashless treatment of up to ₹5 lakh per family per year at empanelled hospitals across India. Beneficiaries can apply for the Ayushman Card online through the PMJAY portal, Ayushman App or Common Service Centres (CSCs) after Aadhaar-based verification.

Who Can Apply for Ayushman Card?

Ayushman Bharat PMJAY eligibility is determined using the SECC 2011 database, NFSA beneficiary data and approved occupational categories in eligible states.

The scheme mainly covers economically vulnerable families, disadvantaged rural households and low-income occupational groups in urban areas. Additionally, all senior citizens aged 70 years and above are eligible under the scheme irrespective of income.

Documents Required to Apply for Ayushman Card Online

The following documents may be required during Ayushman Card verification or registration depending on state-specific implementation rules:

  • Aadhaar Card for identity verification
  • Aadhaar-linked mobile number for OTP authentication
  • Ration Card or Family ID for beneficiary verification
  • PAN Card or alternate identity proof
  • Income certificate, if applicable
  • Caste certificate, where required
  • Passport-size photograph
  • Family relationship proof for family-based verification
  • RSBY URN or state-specific beneficiary documents where applicable

How to Apply for Ayushman Card Online?

Eligible beneficiaries can complete Ayushman Card application online through the NHA-PMJAY beneficiary portal or the Ayushman App. Application refers to the manual process of adding a new individual or family to the beneficiary list, which is typically handled offline through State Health Agencies or local Ration Offices.

I. Through NHA-PMJAY Beneficiary portal

Step 1: Visit the beneficiary portal

Step 2: Log in as a 'Beneficiary' by entering the registered mobile number and the OTP received.

Step 3: Select the State, Sub-scheme (e.g., PMJAY), and District. The ‘Search By’ option allows individuals to search beneficiary details using Aadhaar number, PMJAY ID, Family ID or name.

Step 4: Once your name appears, click the ‘Do e-KYC’ button to initiate the application. 

Step 5: To authenticate, select the ‘Aadhaar OTP’ option. Your Aadhaar number will be visible. Click ‘Verify’. 

Step 6: Tick the consent and click ‘Allow’. Enter the Aadhaar OTP and mobile OTP. 

Step 7: After successful verification, the system will process your e-KYC. Once approved, click the ‘Download Card’ button to save your digital Ayushman Card.

Note: While most beneficiaries can download the card immediately, it may occasionally take 15–20 minutes for the download link to activate on the portal.

II. Through Ayushman App

Step 1: Download the official Ayushman App from the Google Play Store or Apple App Store.

Step 2: Open the app and sign in using your mobile number and OTP verification.

Step 3: Search beneficiary details using Aadhaar number, PMJAY ID, Family ID or location-based search.

Step 4: Complete the e-KYC process using any available verification method such as Aadhaar OTP, Face Authentication, fingerprint scan or iris authentication.

Step 5: Capture a live photograph through the app for identity verification. The image should match the Aadhaar details.

Step 6: Verify the entered details, submit the application request and download the Ayushman Card after approval.

How to Apply for Ayushman Card Offline?

Beneficiaries who cannot complete online registration can apply for Ayushman Card through Common Service Centres (CSCs) or empanelled hospitals.

I. Through CSC Centres

Step 1: Visit the nearest Common Services Centre (CSC).

Step 2: Request for the Ayushman Bharat Card and provide your contact number for verification.

Step 3: Submit the relevant documents. 

Step 4: Once your documents are verified successfully, an authorised individual will be assigned to issue your Ayushman Bharat card.

Step 5: Pick up the card from the specific CSC after a few days.

II. Through Empanelled Hospitals

Step 1: Visit a PMJAY empanelled hospital with Aadhaar Card and beneficiary details.

Step 2: Contact the Ayushman Mitra helpdesk available at the hospital.

Step 3: Complete beneficiary verification and Aadhaar-based authentication.

Step 4: After approval, the Ayushman Card can be generated for eligible beneficiaries.

How to Apply for Ayushman Card for Senior Citizens?

Here is a step-by-step guide to the process:

Step 1: Visit the official PMJAY website.

Step 2: Click the ‘Am I Eligible’ link from the top menu.

Step 3: Enter the captcha code and mobile number and click ‘Verify’.

Step 4: Enter the OTP and captcha code and click the ‘Login’ button.

Step 5: Select the ‘Click Here to Enroll’ button.

Step 6: Enter the Aadhaar number and captcha code and click ‘Search’.

Step 7: Click on the ‘Click Here for fresh enrollment of 70’ link.

Step 8: Select the ‘Aadhaar OTP’ option to complete e-KYC process and click ‘Verify’ against your Aadhaar number.

Step 9: Tick the consent box and click ‘Allow’. Enter the Aadhaar OTP and mobile OTP, select whether you are a beneficiary of any other scheme, and click ‘Proceed.’

Step 10: Click on ‘Capture Photo’. Your photo will be captured.

Step 11: Next, enter the additional information, family members, and click ‘Submit’ to complete the enrollment process.

How to Check Ayushman Card Status Online?

Follow the step-by-step procedure below to check the status of your Ayushman Card online:

Step 1: Visit the official PMJAY website.

Step 2: Click the ‘Am I Eligible’ link from the top menu.

Step 3: Enter the captcha code, mobile number and OTP for verification.

Step 4: Enter the captcha code and click the ‘Login’ button.

Step 5: Choose the ‘State’, ‘Sub scheme’ as ‘PMJAY’, ‘District’, ‘Search By’ and enter the required information and click ‘Search’.

Step 6: Your name will be displayed if you are a beneficiary under the Ayushman Bharat Yojana. The Ayushman Card status will be displayed against your name.

If there is ‘Do e-KYC’ button against your name, it means you need to complete the e-KYC to download your Ayushman Card. If there is ‘Download Card’ button against your name, it means the card is ready, and you can download the Ayushman Bharat Card.

How to Download the Ayushman Card Online?

Eligible beneficiaries can download the Ayushman Card online after successful e-KYC verification.

Step 1: Open the beneficiary portal and log in using mobile OTP.

Step 2: Search beneficiary details using Aadhaar number, PMJAY ID or Family ID.

Step 3: If the e-KYC process is completed successfully, click on the ‘Download Card’ option.

Step 4: Complete Aadhaar OTP authentication if prompted.

Step 5: Download and save the Ayushman Card PDF for future use.
