The Aadhaar card is an essential document one must have in recent times. It is a proof of identity and proof of address; mostly, it is a source to identify you as an Indian citizen. If you still don’t have an Aadhaar card, it is high time for you to enrol for one.
Step 1: Download the Aadhaar enrolment form from the UIDAI website. You can also visit the nearest Aadhaar center to get the form. It is a two-page document – the first page has nine fields to be filled and the second page contains the instructions to be followed to fill the form.
Step 2: Fill in the form with the necessary details. Follow the instructions given in the form to avoid mistakes.
Step 3: Attach the relevant supporting documents to prove your identity, address, and date of birth. These documents must be valid and accepted by UIDAI. If you are using an introducer or Head of Family (HoF), provide proof of relationship with the member.
Step 4: Visit the nearest Aadhaar center and submit the form along with the supporting documents.
Step 5: The staff at the center will take a photograph of yours and capture the biometric data such as iris and fingerprint. This data will be added to the database along with your details provided in the form.
Step 6: Collect the acknowledgement slip that contains your details, enrolment ID, and the date and time of enrolment.
Step 7: You can track the status of your Aadhaar card with the enrolment ID.
a. Use capital letters: Fill-in your details in the form using capital/uppercase letters. It is easier to read and distinguish between letters with uppercase alphabets. The probability of an error in your name, address, date of birth, and other details is lesser in this case as compared to lowercase joined letters.
b. Keep the form neat: Try to keep the form neat and avoid any ink blots on the form. If the information you have provided is not readable, it will affect the information that goes on your Aadhaar card.
Full Name: Enter your full name i.e. first name, middle name, and last name. The name you enter here must match with the name on your proof of identity.
Gender: Specify your gender – male, female, or transgender.
Age/Date of Birth: Provide your date of birth in the format DD/MM/YYYY (date-month-year). If you are unsure of the exact date of birth, you can specify an approximate age in this field. If there is no proof of date of birth, tick the box with the label ‘Declared’. If you have supporting documents, tick the box with the label ‘Verified’.
Address: Enter the postal address on the form as specified in the address proof. You must know that the card will be delivered to the same address you provide in this field. Also, provide your email address and mobile number within the same field.
Details of Family Members: You can provide the name and Aadhaar number of your family members in this field. The family member, here, can be your father, mother, husband, or wife. It is mandatory for children below the age of 5 years to provide the details of parents/guardian.
Consent: Tick the box that says you agree to allow UIDAI the right to share your information including biometrics for the generation of Aadhaar and authentication. provided in the form with certain agencies that deliver public/welfare services.
Documents Attached: The information in the form can be verified based on the documents provided by you, If you have provided details of the introducer or HoF. Tick the box according to your case. Also, you must mention the documents you are providing with the form – proof of identity and proof of address. Proof of relationship and proof of date of birth can be listed if they are applicable to you. If you have provided details of introducer/HoF, the documents can be taken from their file for verification.
Signature: Sign the document at the designated area on the form to show your consent. Alternatively, you can also place your thumb impression. The same form is valid to make changes in an existing Aadhaar card; may it be correcting/updating details. For this, you must provide your 12-digit Aadhaar number. Further, you must only fill in details that must be changed in your card.
